Create your custom ratio using the top drop down option 'Add Aspect Ratio...' under the Crop icon.  For example, if you wish to print an image on 5x7 paper, create a custom ratio of 4.7x6.7 and label it something recognizable.  Since the opening on a ready-made matte for a 5x7 picture is usually 4.5x6.5, this method allows 0.1 leeway around the image for centering within the matte.  Of course, you can adapt this strategy for your particular circumstances.

I use CO20 to produce a TIFF file and export that file to Epson Print Layout.  There I select the paper size, orientation, Centering (Center), Layout (Image Size), Lock Image ratio then tweak the Length and Width sliders to 4.7 and 6.7 (they will be in sync).  Select your Color Settings and you should be all set to print.  You can save these adjustments as a preset for further simplicity and streamlining.
