Rename Catalog

To rename a catalog is it enough to just rename the database file (and folder if I wish) or do I have rename all the items in the Capture One Catalog folder?

Are catalog folders always created in the location where the images are found?

Under the Folder view is it possible to have the image count in the catalog to show all the entire count of all images in the folders under the catalog? See screen shot please.
