Can you confirm that hou have made all capture folders a session favorite?

I'm having the same issues too.

I generally use the presets e.g. only red tags.
The only way to get all of the tagged images into the smart album reliably is to close and re-open the session.
Even then, when more tagging is done, some of the earlier tagged images drop out of the smart album.

    (Yosemite, 8.3.3 etc)

It points to a corrupt session database. Try the verify command from the File menu.

I tried this. Along with a clean (+terminal) delete/reinstall, mac restart, alternative user, permissions verify.

I then loaded three CF cards into three separate folders, all in favourites, and did some random tagging across all three folders.
The first folder showed in the smart album, the other two didn't.
That was last night. This morning I restarted and then relaunched the session and only 8 of 22 tags showed in the smart album. I added another folder and tagged some more images, which didn't show in the smart album.

    Anything else I can try?

I have the same issue with every new session I create. The only workaround I have found so far. Is to quit Capture One every time you experience the problem and restart it. The smart albums should work again. I think it's a bug in CO which has been around for a while now and has not been addressed.
