Check out the Import panel. Half way down the left side is a Naming tab with lots of choices. Click on the choices you want, add your own punctuation to separate them. As always, use a small sample to test. Hope this helps.

That's what I've done. As noted, I don't see an option for the numbers in the cameras filename. Am I missing it? What's the token called for it?

I don't think you can do this on import. But what you can do is rename the files after import. So you would get them named on import as YYYYMMDD - Camera Serial Number - IMG_####.cr2 using the tokens for date, camera serial no and file name, as before. Then you can use batch rename to rename them as YYYYMMDD - Camera Serial Number - ####. The batch rename tool is on the File menu. One of the options is find and replace (the other is using tokens). You could find IMG_ and replace it with nothing. (I've just tried a similar thing on files in my session trash folder and successfully renamed a batch of images with names like 17-04-18-0023.ARW to 17-April-18-0023.ARW by replacing -04- with -April- )

Depending on your camera make/model, you may be able to successfully utilize the Camera Counter token for this purpose.
