Outputting from starred folder, additional files being added

Documentary shoot of 2500 images. First round edit to one star rating of 950 files. Second round edit of that one star folder down to 725 2 star files. Send 2 star files to client as comps. Problem is, when I output the 2 star folder (725 files), actual number of files output is 743. Somewhere along the line, 18 one star files that were rejected in the second round of editing have been added to the output list and sent on to client who, of course, chooses a rejected file as one of their hero shots.
Has anyone else had this happen? I'll admit, it could be user error (though I'm not quite sure how), but this isn't the first time I've seen this. It's happened 3 or 4 times now.
Any thoughts?

How are you selecting the star groupings; the Filters tool, a Smart Album, or otherwise? 0
