Gradient mask 'lock'
Holding down SHIFT while creating a gradient mask should constrain the mask to 45º increments?
Whenever I try this it is about 10º off. I can never get it true vertical (or horizontal).
Has anyone else experienced this? Has anyone found a fix?

release the mouse before releasing the shift key.

It makes no difference what I release first. As I am dragging down (from the top for example) while holding SHIFT, the lines (and red mask) are about 10º off horizontal

Did you disengage "shift lock"?

I don't use shift lock

Me neither… ever but my big fingers are at times clumsy

Strange indeed. I think I will get used to it. I've come from Lightroom Classic where it works spot on, so it's bugging me at the moment.
It's about 10º off, but now you've asked, it varies from image to image.
I've noticed that it shows up in some of the examples in the CO manual.

I have exactly the same problem. It comes and goes randomly
