How do I submit a request
If I click on Submit a request no form loads.

Looks like there is a form hidden below many of the items in a long list of topics common for frequently asked questions. Pick and open the relevant topic. You will see one or more links to articles Capture One staff thinks you should read, and potentially a form where you can submit a request if you do not find the information you need in linked article(s).
The intention, I believe, is to reduce the amount forum traffic where new users ask the same question over and over without first consulting the FAQ nor the collection of relevant articles/videos available.
If none of of the listed topics fits the topic at hand, check out the "Other" near the bottom.
