The instability of using a large Capture One Catalog has long since been documented as has the duplicate versions of C1 version 11.

If you rename a file, but then check the original file in explorer, you’ll see that it doesn’t really rename the original unlike Lightroom which does. Yet another Capture One bug.

I've had the same problem as the original poster as well. C1's catalog is a mess!
