Developed status not always triggered

In 5.1 I've noticed that the developed 'gear' icon is sometimes not showing for images I've developed. Seems to mostly happen if I develop a number of images one after another manually. They are showing up in the developed history list, but no indication they have been developed in the thumbnail (grey gear icon on image) or in the list view column.

  Where are the images (and sidecar) files located? Where is the session?
  On an external source?
  
  What if the files are packed as EIP?

  Images (Canon 30D .CRW) are on same HDD as OS and Program Files.
  
  The sidecar (if you mean .cop, .cof, .cos files) are in default directories automatically reated by 5.1.
  
  The develop directory is a directory called 'Develop' I created as a sub directory from where the .CRW files are (just as 3.x used to to automatically)
  
  This is exact set up I had for 5.0.2 which never had this issue.
  
  I will try to pack as .eip and see what happens.
