It sounds like you have a Subfolder field (either in the Output Location or Recipe's file tab) set with an [Image Name] or other token. Clear the subfolder field to prevent the subfolder creation.

No, but you could co-opt the Focus tool to be an additional "preview" window, zoomed just to the area needed. Just float the Focus tool over to the on-set monitor and resize it to your needs.
