Have you tried to remove your preferences and session files etc? I believe there is a knowledge base article on that somewhere.

Make sure you don't have the Canon capture software running at the same time as it will fight with Capture One over who should connect to the camera.

You can add it from the view menu or by right clicking the empty space in any tab. Of course you can also just organize the tools the way you want them 😉

If you drag the viewer to the second monitor, it should show up there the next time you open it. Is this not the case?

It might be related to the size of the collection you view when you open CO. If you start up in a large collection, CO will take longer to load. Also, if you have many albums or favorites, loading ...
