Importing from Card reader - How?

I realise that most of the problems brought up on this forum are complex or just plain odd because most of you know what you are doing. So apologies for what follows but I do need a bit of help.

When I first started using C1 I was importing from hard drives with my library already laid out as I wanted and the importing was faultless once PO had pointed me in the right direction.

I now want to start importing from my card reader, with the originals going onto an external hard drive, the previews into an existing catalogue with the file name matching that on the hard drive and the image files retaining the cameras numbering.

Would someone please walk me through this. I am not particularly dim but for whatever reason I am finding this simple task a complete impossibility. 😕 😭
A screen grab of the import page set as per my needs would be good but whatever advice i can get will be greeted with joy unlimited!!

I did say it was simple didn't I.

Well simple if you know the answer.

  You launch the Importer (File>Import Images or press "I"), you select the Card in the first menu item "Import from" and that's it.

  Hey there and welcome
  if you can't see the card you might have to change the settings in your camera
  like my Nikon i had to change the storage settings

  How would changing in-camera settings help with getting a card reader to work, Henrik?

  Well when i pluged In my card into the reader
  I could not see it
  But if i pluged In my Nikon i could see it
  Then i changed the setting In my camara then i could use the card reader
  This was first on my d200 and then
  My d3

  Thank you all , so far.
  
  Having opened the import page and found the card and selected it as the 'import from'.
  
  Then what do I put in the other boxes.
  
  The titles in their boxes do not mean anything to me.
  
  What goes where to give me the import that I described in first post.
  
  This will really help me!

  I have a screen grab of this and have tried to follow it a dozen times without success.
  
  Obviously I am reading it wrong. But where?
