With the latest version sometimes exporting to photoshop gives bad results, The image is spitted in 4 parts and 1 part is bad. It's annoying when you have little time to deliver several photos for a model agency.
Only when I exit CO1 12 and run CO1 again the conversion works without problems. Seems I have to close CO1 12 sometimes.
left: RAW
middle: final PSD
right: error after export ---to PSD (see forehead)
ps. I love the capture one software. Every RAW file is processed by CO1 and finalized after Photoshop in CO1.

That looks like either some sort of problem with cache memory/disc file management (or maybe some other sort of temperamental and inconsistent hardware issue) or something to do with GPU based hardware acceleration.
Have you already tried running the output process with Hardware Acceleration turned off in Preferences?
If not that would be worth a test to see if it make any difference to the processes success rate.
If it does then is may be worth checking the GPU driver provider web site for updates (or, sometimes, downdates!).

I will try that but I always have the latest drivers installed. Never had this problem with CO1 11.0 -

There have been one or two occasions when I got a better result from an "earlier than latest" driver.
Usually the Windows drivers and PC manufacturer's drivers need to be checked carefully for real age.
That said I am not totally convinced that the GPU firmware/driver search engines are entirely accurate as I have, from time to time, arrives at a completely different release of driver depending on where and how I searched.
Perhaps I have been unlucky?
I would suggest looking at the log files to see if the system is self reporting anything.
Also create a Support Case to let Phase look at the files - but take into account the time of year in terms of response expectations.
