I have no idea why it would be unreliable but you can manually trigger the update from files with the Image | Load Metadata command.

Burried on the preferences help page (https://support.captureone.com/hc/en-us/articles/360002544898-Metadata-in-XMP-files) is this:
" Using Full Sync and working with a large collection of new images can hinder performance. In this situation, it's recommended to use the "manual" sync commands, Image > Sync Metadata and Image > Load Metadata. This will achieve the same end result, but without the background sync process slowing things down."

I had a devil of a time finding that - I opened a support request about the issues I found with their search.

    Ian,

I was search for something yesterday in relation to a Knowledge Base article (Phase One camera side of things) that had been linked to in another post. No matter what I tried the P1 KB search did not find it in the early hours of this morning - even search by the full KB number.

Having seen you comment I was I was going to go ahead and report the problems as I had decided to do ... but ran one more check and up came the KB post Not searched for by KB number) at the top of the list.

So I guess either the document set has been re-indexed in the past few hours or there is a random search problem in the KB system.

I'll keep monitoring and see what happens before raising a case.

In my case the first set of search terms were a bust (I get that sometimes happens) it was the second set that caused me to feel WTH. Those terms turned up an hit that looked nearly perfect but when I clicked on the link it provided it linked too what turned out to be the parent of the search result instead of the actual article it found. So, looks like a problem of the search results linking to the wrong place. I had to dig for the actual content and search on the page for the text in the result summary for each of the linked child pages until I found it. Not a good search experience. Likely a fixable one though.
