Creating Hierarchical Keywords

I don't know how to create hierarchical keywords as there is no Plus sign or any other icon on the hierarchical keywords bar. Am I missing something or do I have a corrupted program?

  On my Windows system, there is a -+ next to the left had menu item "Heirarchical Keywords
  
  
  
  I add the 1st key word by hitting the plus, then the sub word by right clicking on the word.
  
  You should be able to do the same.

  There was no -+ on my Hierarchical keywords bar and I was about to do a screenshot to show it but as I was preparing to do it, I collapsed the keywords bar and then I noticed a large blank space between the collapsed Keywords bar and the Hierarchical keywords bar. I found if I dragged the Hierarchical keywords bar upwards, low and behold, suddenly the -+ appeared!
  
  So I'm good now.
