export is duplicating output files

I star images I want to output, I select all with 5 star, choose folder, output in jpeg to that folder and when the software outputs the files they is one duplicate of each image...WHY & how do I fix this? It has just started doing this and I havent upgraded or installed any updates since purchase....

also, how do I clear my history in the inport to locations & Session Favorites?

  Make sure you don't have more than one process recipe selected.
