    This is general communication error. Sometimes firmware related and other times in-camera setting.

    Von - there is a plastic FW adapter that should make the cable sit more tight . Unfortunately in my opinion it doesn't work that good (= it is a piece of crap , but maybe it works for you ) .

    Should be available through your Canon dealer .
    I got mine for free ( costs around 4 $ ) .

    I'm very familiar with the problem you describe .

    It made me almost mad some months ago .

    I made a recent shoot with the same set up tethered to a intel MacBook Pro , and
    didn't get any lost connections at all , so strange enough , on older computer there must be some software issue above sensitivity for looose cable .
