Action Log errors

I have been updating a catalog by right clicking a directory and selecting 'update now'. Every time I do though the action log pops up at the end and displays a number of files that it says "Unable to scan media file" in regards to. Exactly how do i determine what went wrong and how do i fix it? There doesn't seem to be anything wrong with the files that were not able to be scanned as they all are readable in other applications with out issue.

Not sure what to do now or how to proceed to get these into the catalog.

Also, is it possible to have the date displayed on the errors in the action log not just the time of the error?
