Could you check the settings in the Base Characteristics tool, the ICC profile and the curve? The error you mention can sometimes occur when there is a mismatch in this tool.

About the PSD issue, consider TIFF instead. I think you lose nothing and gain a lot.

Naming layers is key. Names like 'sky - darkened' or 'eyes - sharpened' help to keep oversight. I sometimes refer to tools as well like in 'shadows - WB' for White Balance.
